As per, the British Columbia provincial safety regulator Work Safe BC (WSBC), OHS Regulation Part 22, Underground Workings, s. 22.6 Notice of Project, (1) "No later than 30 days before commencing an underground project, written notification of the project must be given to the Board by the owner or another person engaged by the owner to be the prime contractor, except that if there is more than one prime contractor employed on the project the notice must be given by the owner." - file for review, the Notice of Project package as submitted to WorkSafe BC, prior to commencing the construction of the Burnaby Mountain Tunnel.

Per the Trans Mountain Expansion Project's (TMEP) contractors requirements, a Contractors Hazard Risk Registry (CHRR) is required to be developed, submitted and maintained. TMEP then combines the individual CHRR's into the project wide Unified Hazard Risk Registry (UHRR).
File for review, the latest version of the CHRR and or UHRR, applicable to March 2021 operations at Burnaby Mountain Terminal or Westridge Marine Terminal.

Observation 3 - Information Request - 3 - High Voltage/ ARC Flash prevention and protection AND the electrical isolation procedures as applied to the Tunnel Boring Machine's tethered power supply.
Inspection Officer observed the 12.5 kV electrical supply system to be used to power the tunnel boring machine and it's associated operations.
File for review, the applicable and comprehensive high voltage isolation, ARC flash prevention program or procedures, inclusive to lock-out/tag-out and isolation of the applicable electrical conductor and cables - as used to power the Tunnel Boring Machine.
