Safety oversight of construction activities - worker safety [Construction inspection of the NGTL McLeod River South Project. The Project consists of two non-contiguous pipeline loop sections of Nominal Pipe Size (NPS) 48 pipeline that will loop the existing Grande Prairie Mainline (GPML) and GPML Loop. GPML Loop No. 2 (Bronson Section): The Bronson Section is located approximately 80 km northwest of Edson, Alberta and consists of approximately 12 km of 1219 mm (NPS 48) outside diameter (OD) pipe. GPML Loop No. 2 (Hornbeck Section): The Hornbeck Section is located approximately 40 km northwest of Edson, Alberta and consists of approximately 23 km of 1219 mm (NPS 48) OD pipe Assess compliance with the OPR (Safety Management Program), and construction safety manuals/project specific safety plans.

Observation 2 - NNC #1 - Safety Data Sheets (Banister Yard Warehouse)

Material Safety Data Sheet Binders in the warehouse contained Safety Data Sheets (SDS) that had expired and also contained sheets for products that were not present on the project.

Overhead powerline voltage was not identified on signage as required in the Construction Safety Manual (Site Specific Safety Plan) filed for the project.

Observation 4 - CNC #2 - Unmitigated Tripping Hazard (Banister Yard)

A storm drainage pipe was observed along the side of the warehouse buliding that needed infilling and/or barriers to prevent the potential for personnel to step in to the hole observed around the drain pipe.
The company shall ensure that the hazard is controlled by adding a barricade or by filling in the hole observed around the drainage pipe.

Observation 5 - CNC #3 - Improper Grounding (HDD site, SF Road 6)

Ground rods were not penetrated in earth to obtain a ground nor were the HDD generator ground cables connecting securely to the rods.
Company shall install ground rods per the Canadian Electrical Code (CEC).
Observation 6 - CNC #4 - Improper Toe Board on Elevated Work Platform (HDD Site, SF Road 6)

A temporary work platform (the Centrifuge Platform) at the HDD site was observed to not have a toe board in place that met the requirements. It was also observed that the platform lacked a barricade across the opening at the top of the stairs to prevent falling.
The company shall ensure that corrections are made such that the elevated work platform includes toe boards and barricade across the access opening as specified in the Occupational Health and Safety Standards for Prime/General Contractors (Cdn-US-Mex).

Observation 7 - CNC #5 - Worn Rigging Slings (HDD Site, SF Road 6)

Rigging slings were observed in the HDD rig that demonstrated significant wear/fraying such that they should not have been available for use.
Remove slings that were observed as having significant wear/fraying from being available for use.
